Transitions out of School for Persons with Disabilities

     

As important as it is for our students to have proper transitions into school it is for them to have transitions out. Often times at the age of 21 when a student graduates from their dev ed or life skills class parents are left wondering what to do next. The following is a number of community connections for families with children with disabilities.  As well in the chart are work and day programs available for theses students, respite and residential care, personal and in home supports as well as some recreational options to keep busy. These options are available throughout the region for adults with mental and physical disabilities. Making the transition into their adult lives possible.

  1. There is a student at the school I had my last placement at, who is too old to continue going there, but has been allowed to make monthly visits to the class. This experience has been amazing for both her and the other students who adore her and look forward to her coming to spend time in the room. I really commend the school for allowing this because of the positive impact it has on everyone involved.

  2. In the 7th and 8th grades, I took the city bus to school. That particular route went right past ARC Industries (http://arcindustriescambridge.ca/about/ it's run by Community Living) in Cambridge. It provides work for adults with disabilities, and many students who 'age out' of the school system transition here by doing work placements through school. The people who took the bus with me where wonderful, and they all still remember and say hi to me when they see me out and about. Most still work there today, and all of them love their jobs!

  3. it is very important to have a transition plan for students leaving the school system with an exceptionality. Successful transition from school is based on the students:
    1. Identified needs
    2. Recongized strengths, skills, and compentencies
    3. Long term and short term goals
    4. Past experiences in the community
